Preparing for adulthood (PfA) - Transitions

Find out about the pathways in preparing young people with special educational needs and disabilities (SEND) and other additional support needs for adulthood.

Hammersmith and Fulham Local Area Partnership is committed to helping young people lead happy, healthy, and fulfilling lives and achieve their life goals through provision of high-quality care and support. We want to ensure young people are supported to gain independence and experience a smooth, seamless, and person-centred transition into adulthood.

What is preparing for adulthood?

The term 'Preparing for Adulthood' (PfA) describes the pathways in preparing young people with special educational needs and disabilities (SEND) and other additional support needs for adulthood, enabling them to gain the necessary independence skills to do so.

Preparing for adulthood means preparing for:

  • further or higher education and employment – including options for supported internships, apprenticeships, self-employment, volunteering, college and university
  • independent living – this means young people having choice, control and freedom over their lives and the support they have, their accommodation and living arrangements, including supported living as far as reasonably possible
  • participating in society, including having friends and supportive relationships, and participating in, and contributing to, the local community
  • being as healthy as possible in adult life.
