Principles of a good transition

Transitioning from children's to adult services can be challenging for young people and their families. We aim to support, inform, and empower them during this process. Early identification and support are crucial to ensure adequate care and preparation.

Collaboration and communication among teams, young people, families, and other organisations are essential.

Our goals:

  • Start early to develop independence skills.
  • Promote independence and community involvement.
  • Focus on person-centred support and outcomes.
  • Ensure smooth transitions with integrated, multi-agency cooperation.
  • Provide choice and control for young people and families.
  • Use strength-based principles, considering both formal and informal support networks.
  • Ensure a robust transition process and address any gaps with co-produced plans.

Co-production

We value the voices of children and young people in shaping our services. Our Local Area Coproduction Framework guides our partnership with young people, families, and other stakeholders to create shared solutions. Each young person's transition plan is personalised to their unique circumstances and aspirations.

SEND outcomes framework

We have identified five key outcomes for children and young people based on their feedback.

  1. My voice is heard, valued and acted upon
  2. I am able to learn
  3. I have choice and control
  4. I feel happy and enjoy good health and well-being
  5. I feel safe, supported and part of my local community

Our SEND Outcomes Framework helps us measure our effectiveness in supporting them, ensuring their voices are heard, and improving our practices and services.

During the transition process, there are various reviews, assessments, meetings, and interviews based on the young person's needs. Some are repeated, while others happen once. These steps, including legal requirements, ensure the young person and their family are involved and that the care provided meets their individual needs and preferences.
