The Family Hub Stephen Wiltshire Centre is a specialist centre for children and young people (0 to 18 years) with special educational needs and disabilities (SEND) and their families.
Our services are only available to people living in Hammersmith & Fulham.
If you live outside of the borough
If you live outside of the borough, and you are interested in visiting the centre for any of the activities, events or services and, contact your local authority who may be able to fund sessions for you to attend.

What we offer
We offer:
- information, advice and support, related to SEND questions/queries every Tuesday (term time) 10am-2pm with a Family Navigator
- drop-ins and group sessions delivered in partnership with local services and agencies
- fully accessible building and toilets
- services that support families to meet professionals and peers
- a range of other services, including meeting the Short Breaks Review Team
- short breaks including specialist disability after-school clubs, weekend and holiday play schemes.

Register with family hubs
You need to register with us before you can use any family hub services or attend an activity or session. To register you can:

Booking sessions for 0 to 5-year-olds
You must book in advance to attend any sessions for 0 to 5-year-olds. These activities are not available to childminders and nannies due to limited spaces for families.

Health visitor assessments
Mondays, 12 noon to 2pm
New-born to 4 years
Health visitors support families with children from newborn to 4 years. Sessions will have either a walk-in clinic for weight checks or discussion about your child's development.

0-5 Little Explorers (stay and play)
Mondays, 12 noon to 2pm
0 - 5 Little Explorers is a drop-in stay and play session for parents with children up to 5 years old with SEND. Health visitors attend our stay and play sessions so are able to speak with parents during this time.

CAMHS Clinic: Support and advice for professionals, young people and families around Mental Health
Mondays, 2pm to 4pm
CAMHS offers support and advice for professionals, young people and families around Mental Health. If you wish to speak with someone for information and advice, please contact SWC Duty to book an appointment. If you wish to be referred to CAMHS, please speak with your GP or your child's school.

0-8 Little Explorers (afterschool stay and play)
Mondays, 3pm to 6pm
0 - 8 Little Explorers is an after-school drop-in stay and play session for parents with children up to 8 years old with SEND.
Health visitors attend our stay and play sessions so are able to speak with parents during this time.

Fulham Football Club Junior Pan DisAbility are sessions for children ages 5 to 16 with any form of physical and/or learning disability or special educational needs.

Perinatal Mental Health Service – weekly (invite only)
Tuesday, 9am to 1pm
Our perinatal mental health service is a dedicated team here to support women experiencing mental health challenges during pregnancy or after the birth of their baby. We understand that this period can be especially difficult, and mental health difficulties are common. Our goal is to help you feel as well as possible during this time so that you can enjoy your pregnancy and family life.
You may benefit from our service if:
- You are experiencing moderate or severe mental health difficulties, either currently or in the past.
- Your mental health concerns are not improving with the support of your GP.
Booking required: Please complete the self-referral form to schedule an appointment.
Early years Speech and Language Therapy Groups
Tuesday, 9am to 2pm
Speech and Language Therapy Groups are run by the NHS CLCH Early Years Speech and Language Therapy (SLT) Team. The groups support children who have been identified as having language and/or communication needs and their parents/carers to develop communication skills. Parents can self-refer to the SLT service or can request a referral through GP/HV, family support worker, social worker or nursery. Parents cannot self-refer or drop into groups as the children need an assessment first.

Housing Clinic is run by staff from Hammersmith & Fulham Council Housing.

Oral health support
The final Thursday before each half term, 10am to 11:30am
Central London Community Healthcare Oral Health Team visit Stephen Wilshire Centre to offer free advice and resources to raise awareness around the importance of good oral health focused on children with special needs.

Parentsactive

Pip squeak
Monthly, Thursdays, 3pm to 6pm
Pip Squeak is a stay and play drop-in group for pre-schoolers with hearing loss and their parents and carers. You will be able to meet other parents and carers as well as getting advice from a teacher of the deaf as well as a Speech and Language Therapist.

Welfare benefits advice
2nd and 4th Fridays of the month, 9:30am to 12 noon
These drop-in sessions are for families of children with disabilities or special educational needs.

Cygnet training
Fridays, 10.30am to 1.30pm
Cygnet Training is for parents of children with autism aged 5 - 17 years old, living in Hammersmith and Fulham.

Short breaks
We deliver short breaks to children after school, at weekends and during school holidays. Depending on the needs of the child this support may be accessed without a needs assessment.

The Children's Disability Register
We hold a register of disabled children. You do not have to register your child but it can have benefits if you do.

We want parents and carers to be involved in the development of the centre and we welcome any feedback.
