Disabled Children's Team

We work with children who have severe and permanent disabilities and/or health conditions. We support families who may require additional help to meet their children's needs.

At present the teams have social workers and support staff, occupational therapists and a transitions worker. We also work closely with other professionals from health and education to provide an integrated and efficient service.

The Social Work Team

The Disabled Children's Team provides specialist social work services to disabled children and their families. Services include:

  • Child Safeguarding Services
  • Services to Looked After Disabled Children
  • Assessments to access support packages for children with complex needs
  • Youth hub

The Stephen Wiltshire Centre

The Stephen Wiltshire Centre is a ‘one-stop shop’ for children, young people and families with special educational needs and disabilities (SEND). You can contact the centre on 020 8753 4443.

This service includes:

  • Play schemes after school, weekends and school holidays
  • Stay and Play for families
  • Summer holidays and family day trips

How do you access this service?

For families who want more support, a package level is determined following an assessment by a key worker or social worker (in this context referred to as the assessor). Assessments usually take place once a year. If a family does not want an assessment, they can access the core offer without an assessment. The assessment is made on the basis of two components: the child's needs and the additional care they need as a result of their disability and the impact of the child's disability on the social, sibling and parenting circumstances. These needs are identified through the assessment and panel form.
