The service offers completion of the statutory review health assessments for all Looked After Children aged 5 years and over placed by Hammersmith & Fulham, Kensington and Chelsea and Westminster City Council.
The team support:
- the completion of a health care plan
- liaise with and support named social workers
- offer case working to support health promotion to children and young people not in school
- support foster carers to manage health conditions
- refer to community services as required
- follow up initial health care plan with multi-agencies as required
- complete a summary of health whilst in care to those rising 18 years of age
- support care leavers though a seamless transition to accessing of adult services
