Learning Disability Service

The learning disability service works with adults who have a learning disability. Anyone with a learning disability who lives in Hammersmith & Fulham and has left school can ask for help from the team. Families and carers can also contact us for support.

Our team is made up of a range of professionals who can provide help in different areas. We have social workers, community support workers, physiotherapists, psychiatrists, clinical psychologists. occupational therapists, community nurses and speech and language therapists. We will put each person in touch with the professionals who we think can help them most.

The services we provide include:

- support to make important decisions

- help with getting benefits

- help getting out and about

- support using GPs and other health services

- advice on keeping healthy

- advice on specific conditions such as Down's Syndrome, Fragile X and Autism

- help with mental health needs and emotional problems

- opportunities to meet new people and learn new skills

- help with communications needs

- practical help at home - such as getting a meal

- organising residential or nursing care for people who cannot live independently in the community

- personal Health Action Plans

- help with behavioural difficulties

- advice and support for carers

- organising short breaks for service users and carers.

Anyone can make a referral by letter or telephone. We then visit potential service users at home to assess their needs and talk to their family, friends or other carers.

If someone qualifies for our services we talk to them about the support we are able to offer and work out an individual plan to meet their needs. We can also help you find out more about other organisations who support people with learning disabilities in Hammersmith & Fulham.
