Integrated Key Working Team

Families with children with identified needs and disabilities are offered support from an Integrated Keyworker, as part of the IKW Team. They currently work with children and young people aged 0-13 and their families. They provide co-ordination of services, information and advice, support with accessing Education, as well as advice on accessing funding for nursery or other childcare. They will also help with benefits and financial difficulties.

The current criteria for support from the IKW is where children are accessing:

  • 4 x Health Professionals (not including Health Visitor or GP) – e.g. Community Paediatrician, SALT, Physiotherapist, Neurologist etc).
  • 1 x Educational or Social Care professional – e.g. nursery / Portage / SEN Team etc.

Referrals for the service can be made by parents or professionals by calling the SWC Duty Line on 02087534443 (currently open 9.30-12.30).

Once a referral has been received it will be overseen by a Manager to ensure the criteria is met for support and, upon allocation, an IKW will be in touch with families. Please note we do currently have a wait for support from the service.
